Serebral palsili çocuklarda dizartri müdahalesinde LSVT LOUD terapi yönteminin etkililiğinin incelenmesi

dc.description.abstractAktiviteye bağlı nöroplastisite ilkeleri, dizartrili serebral palsili (SP) çocuklarda terapi protokollerine yön vermektedir. Parkinson hastalığına bağlı ses ve konuşma bozukluklarının terapisi için geliştirilmiş Lee Silverman Ses Terapisi (LSVT LOUD), aktiviteye bağlı nöroplastisite ilkelerini dahil eden yerleşik bir protokoldür. Yakın zamanda tek denekli ve vaka çalışması olarak dizartrili SP'li çocuklarda LSVT LOUD'u inceleyen çalışmalar, dizartrili SP'li çocukların ses ve konuşma özelliklerindeki olumlu terapötik değişikliklere dikkat çekmektedir. Bu çalışma; dizartrili SP'li çocuklara LSVT LOUD uygulanması sonucu ses ve konuşmanın akustik ve algısal özelliklerinin incelenmesi amacıyla planlanmıştır. Çalışmaya ortalama yaşları 114,17±23,25 ay olan 12 dizartrili SP'li katılımcı katılmıştır. Katılımcılara yapılandırılmış bir ev programı eşliğinde haftada birer saatlik 4 seans LSVT LOUD terapi protokolü, ardışık 4 hafta boyunca uygulanmıştır. Terapi öncesinde ve sonrasında katılımcılara terapide çalışılan ve çalışılmayan görevlerden oluşan değerlendirme seansları yapılmıştır. Ebeveynler ve çalışmaya kör dinleyiciler katılımcıların ses ve konuşma özelliklerinin algısal değerlendirmesini yapmışlardır. Çalışma sonucunda; katılımcılarda terapiyi takiben ses ve konuşma mekanizması üzerindeki fizyolojik etkileri temsil eden akustik sinyalin belirli özelliklerinde ve genellemeyi temsil eden cümle tekrarlama görevlerinde olumlu değişiklikler tespit edilmiştir. Ebeveynler ve dinleyiciler katılımcıların ses ve konuşma özelliklerinde olumlu gelişmeler bildirmiştir. Bulgular LSVT LOUD'un SP'li çocukların ses özelliklerinde geliştirici etkiye sahip olduğuna işaret etmektedir.en_US
dc.description.abstractPrinciples of activity-related neuroplasticity guide therapy protocols in children with dysarthria and cerebral palsy (CP). Lee Silverman Voice Therapy (LSVT LOUD), developed for the therapy of voice and speech disorders associated with Parkinson's disease, is an established protocol incorporating the principles of activity-dependent neuroplasticity. Recent studies examining LSVT LOUD in children with CP with dysarthria as a single-subject and case study draw attention to positive therapeutic changes in the voice and speech characteristics of children with CP with dysarthria. This study was planned to examine the acoustic and perceptual properties of voice and speech as a result of applying LSVT LOUD to children with dysarthria and CP. Twelve participants with CP with dysarthria with a mean age of 114,17±23,25 months participated in the study. The LSVT LOUD therapy protocol was administered to the participants in 4 sessions of one hour per week, accompanied by a structured home program, for 4 consecutive weeks. Before and after the therapy, evaluation sessions consisting of worked and unworked tasks in therapy were given to the participants. Parents and study-blind listeners made perceptual assessments of participants' voice and speech characteristics. In the results of working; positive changes were detected in certain properties of the acoustic signal, which represents physiological effects on the voice and speech mechanism, and in sentence repetition tasks, which represent generalization, following therapy in the participants. Parents and listeners reported positive improvements in participants' voice and speech characteristics. Findings indicate that LSVT LOUD has an improving effect on the vocal characteristics of children with CP.en_US
