Palyatif bakım hastalarının ağrıyla baş etmesinde müzik terapinin rolü: Sistematik derleme

Özet
Palyatif bakım ünitelerinde yatan hastalarda ağrı şiddetini azaltmak için müzik terapi farmakolojik yöntemlere ek olarak kullanılan tamamlayıcı ve bütünleşik uygulamalardan biridir. Müziğin palyatif bakım hastalarında ağrı şiddeti üzerine etkisini değerlendiren bu sistematik derlemede, bu konuda yapılmış birbirinden bağımsız çalışmaların sonuçları birleştirilen araştırma bulguları ile değerlendirilmiştir. Derlemede; Ovid, Cochrane Library, Pub Med, CINAHL, Science Direct veri tabanlarında Ocak 2015-Aralık 2021 tarihleri arasında ingilizce yayınlanmış randomize kontrollü çalışmalar üzerinden literatür taranmıştır. Taramalar ve duplikasyonları ayırmak için EndNote (EndNote X8) programı kullanılmıştır. Kalite değerlendirmesi iki bağımsız araştırmacı tarafından yapılmış ve Joanna Briggs Enstitute kalite değerlendirme ölçeği ve PRISMA checklistine göre hazırlanmıştır. Tarama sonucunda palyatif bakımda müzik terapinin ağrı üzerine etkisini inceleyen beş Randomize kontrollü çalışma elde edilmiştir. Dahil edilen çalışmaların örneklem sayısı 48-84 arasındadır. Üç çalışmanın canlı enstrümantal müzik dinletisi, iki çalışmanın da kayıttan mp3 çalar ile (kulaklıkla) yapıldığı dikkati çekmektedir. Çalışmalardan üçünün Visual Analog Skala (VAS) ile birinin McGill ağrı anketi ile birinde de kalp hızı ve kan hacmi parametreleri üzerine sempatik sistem aktivasyonu ile ağrı değerlendirmesi yapılmıştır. Bu çalışmada, palyatif bakım hastalarında ağrı şiddetini azaltmada müzik uygulamasının olumlu etkiye sahip olduğu saptanmıştır. Çalışma sonucunda elde edilen veriler ışığında müzik uygulamalarının ağrı şiddetine etkisini araştıran iyi planlanmış daha fazla randomize kontrollü çalışmaya ihtiyaç duyulmaktadır.
Music therapy is one of the complementary and integrated applications used in addition to pharmacological methods to reduce the severity of pain in patients hospitalized in palliative care units. In this systematic review evaluating the effect of music on pain intensity in palliative care patients, the results of independent studies on this subject were evaluated by the combined research findings. In the review; the literature was searched through randomized controlled trials in English between January 2015 and December 2021 in databases of Ovid, Cochrane Library, Pub Med, CINAHL, Science Direct. EndNote (EndNote X8) program was used to separate the scans and duplications. Based on the Joanna Briggs Institute quality assessment scale and the PRISMA checklist, quality assessment was conducted by two independent researchers. The search resulted in five randomized controlled trials examining the effect of music therapy on pain in palliative care. The sample size of the studies included was between 48-84. It is noteworthy that three studies were conducted with live instrumental music and two studies were conducted with recorded mp3 player (with headphones). Pain was evaluated by Visual Analog Scale (VAS) in three of the studies, by McGill pain questionnaire in one study and by sympathetic system activation on heart rate and blood volume parameters in one study. In this study, it was found out that music therapy had a positive effect on reducing the severity of pain in palliative care patients. Considering the data obtained from the study, there exists a need for more well-planned randomized controlled trials investigating the effect of music theraphy on pain intensity.
