Özet
Yarı kapantılı ses yolu egzersizlerinin uygun egzersiz süresinin belirlenmesi ile ilgili mevcut olan kısıtlı sayıdaki araştırma bu alanda daha fazla çalışma yapılmasını gerekli kılmaktadır. Bu çalışmada ses bozukluğu olmayan yetişkin bireylerde pipet fonasyonu egzersizinin 1, 3, 5, 7 ve 10. dakikalarında ses parametreleri üzerindeki anlık etkilerini değerlendirmek ve cinsiyete göre uygun egzersiz süresini belirlemek amaçlanmıştır. Bu amaç doğrultusunda çalışmaya ses bozukluğu olmayan, 18-60 yaş aralığında 35 kadın (32,8±10,5) ve 34 erkek (32,9±9,8) olmak üzere toplamda 69 katılımcı dahil edilmiştir. Katılımcılardan egzersiz öncesi ve egzersizin 1, 3, 5, 7 ve 10. dakikalarında ses verileri alınmıştır. Alınan ses verileri akustik, elektroglottografik ve işitsel algısal açıdan incelenmiştir. Ayrıca katılımcılardan ilgili egzersiz sürelerinde görsel analog skala aracılığı ile egzersizi uygularken harcadıkları vokal efor ile larengeal rahatsızlık hissine ilişkin bilgi alınmıştır. Akustik değerlendirme bulguları; erkeklerde egzersizin 7. dakikasında F0 parametresinde yükselme ve egzersizin 5. dakikasından itibaren shimmer parametresinde düşme olduğunu göstermiştir. SPL parametresi kadınlarda egzersizin 5. ve 10. dakikalarında, erkeklerde ise egzersizin 1. dakikasından itibaren yükselmiştir. Erkeklerde 5. dakikada AVQI skorunda azalma görülmüştür. Elektroglottagrafik değerlendirme sonuçlarına göre; kadınlarda egzersizin 10. dakikasında ortalama Qx (%) parametresinde düşme, Qx SD (%) parametresinde artma elde edilmiştir. Algısal ses kalitesi kadınlarda egzersizin 3. dakikasında, erkeklerde ise 5. dakikasında optimal düzeye ulaşmış olup egzersiz süresi uzadıkça her iki cinsiyet için de bozulmuştur. Egzersizi uygularken harcanan vokal efor erkeklerde egzersizin 10. dakikasında, kadınlarda ise egzersizin 7. ve 10. dakikalarında artmıştır. Hem kadın hem de erkek katılımcılar egzersizi uygularken hissedilen larengeal rahatsızlığın egzersizin 10. dakikasında arttığını bildirmişlerdir. Değerlendirme bulguları birlikte ele alındığında pipet fonasyonu egzersizinin ses parametreleri üzerinde optimal etkilerin görüldüğü egzersiz süresi sağlıklı kadınlarda 3 ila 5 dakika, sağlıklı erkeklerde ise 5 ila 7 dakika arasındadır.
In this study, it was aimed to determine the appropriate exercise duration according to gender by investigating the immediate effects regarding the time of performance (from 0 to 10 minutes) of the straw phonation exercise. This study included 34 men with average age of 32.88 years (SD = 9.8) and 35 women with average age of 32.80 years (SD = 10.5) without voice complaints or voice disorders. All participants performed the straw phonation exercise for one, three, five, seven and ten minutes. Data collection was conducted before and immediately after each set using sustained emission of vowel /a/ and a sentence. Before the vocal exercise and after each vocal exercise runtime, the voices of the participants were examined in terms of acoustically, electroglottographically, and auditory-perceptually. And the information was obtained from the participants about the vocal effort required to perform the exercise and the feeling of laryngeal discomfort that occurred during the exercise using a visual analog scale (VAS). Acoustic evaluation findings showed that there was an increase in the F0 parameter at the 7th minute of the exercise and a decrease in the shimmer parameter from the 5th minute of the exercise in men. While the increase in intensity was seen in the 5th and 10th minutes of exercise in women, it was observed in men from the 1st minute of exercise. There was a decrease in the AVQI score at the 5th minute in men. According to the results of electroglottagraphic evaluation, a decrease in the average Qx (%) parameter and an increase in the Qx SD (%) parameter were obtained in the 10th minute of exercise in women. While perceptual voice quality was optimal at the 3th minute in women and at the 5th minute in men, it deteriorated at the 7th minute of exercise in women and at the10th minute of exercise in men. While the effort spent while performing the exercise increased in the 7th minute in women, it increased in the 10th minute in men. The laryngeal discomfort felt while performing the exercise increased for both sexes at the 10th minute of the exercise. When the evaluation findings are evaluated together, the exercise time in which the optimal effects of straw phonation exercise on vocal parameters are observed is between 3 and 5 minutes in healthy women and between 5 and 7 minutes in healthy men.
