Özet
Araştırma, hemodiyaliz hastalarında hasta tercihli kulaklıkla Türk musikisi nihavent makamında dinletilen müziğin arteriovenöz fistül kanülasyonuyla ilişkili ağrı ve distres üzerine etkisini değerlendirmek amacıyla yapıldı. Bu araştırma tek kör, randomize kontrollü deneysel bir araştırma olarak gerçekleştirildi. Araştırma, özel bir hemodiyaliz merkezinde Mart-Haziran 2023 tarihleri arasında haftada 3 seans hemodiyaliz tedavisi alan ve dahil edilme kriterlerine uyan 54 hasta ile yürütüldü. Deney (n=27) ve kontrol (n=27) gruplarına hastalar rastgele atandı. Araştırmada blok randomizasyon yöntemi kullanıldı. Deney grubuna üç hemodiyaliz seansı boyunca işlemden 5 dakika önce ve işlem sırası olmak üzere toplam 10 dakika hastanın kendi seçtiği (nihavend makamı) müzikler kulaklıkla dinletildi. Araştırmanın verileri; hemodiyaliz hastalarının sosyo-demografik ve hastalığa ilişkin özelliklerini içeren Hasta Bilgi Formu, Distres Termometresi ve Visual Analog Skala ile toplandı. Kontrol grubuna ise rutin tedavi ve bakım uygulandı. Deney ve kontrol gruplarına müdahale öncesi, 1., 2. ve 3. seanslardan sonra ağrı ve distres değerlendirmesi yapıldı. Araştırmadan elde edilen verilerin analizinde tanımlayıcı istatistikler, bağımlı ve bağımsız örneklem t testi, ANOVA testi kullanıldı. Gruplar arasındaki farkın klinik anlamlılığını değerlendirmek amacıyla, partial eta squared (?p 2 ) değeri hesaplandı. Tüm analizler için istatistiksel anlamlılık düzeyi p<0,05 olarak kabul edildi. Deney grubundaki hemodiyaliz hastalarında 3. seans sonunda VAS ağrı şiddetinin kontrol grubundakilere göre önemli düzeyde azalma olduğu saptandı (p<0,001). Deney grubundaki hastaların kontrol grubuna göre distres düzeylerinin azaldığı tespit edildi (p<0,001). Müzik uygulamasının yaşam bulgularını değiştirmediği bulundu (p>0,05). Araştırmadan elde edilen sonuçlar doğrultusunda hemodiyaliz tedavisi alan hastalarda müzik uygulamasının arteriovenöz fistül kanülasyonuyla ilişkili ağrı ve distres düzeylerini azaltmak için güvenilir ve maliyet etkin bir hemşirelik müdahalesi olarak kullanılabilir. Bu randomize kontrollü deneysel araştırma ClinicalTrials.gov'a kayıtlıdır (2022/802).
The research was carried out to evaluate the effect of music played in nihavent makam of Turkish music with patient-preferred headphones on pain and distress associated with arteriovenous fistula cannulation in hemodialysis patients. This research was conducted as a single blind, randomized controlled experimental trial.The research was conducted at a special hemodialysis centre with 54 patients who received 3 sessions of hemodialysis therapy a week from March to June 2023 and met the criteria for inclusion. Patients were randomly assigned to the experiment (n = 27) and control (n = 27) groups. Block randomization method used in research. For three hemodialysis sessions, 5 minutes before the procedure and for a total of 10 minutes in order of procedure, the patient's self-selected (nihavend office) music was played with an MP3 headset. The control group underwent routine treatment and care. Before intervention in the experimental and control groups, 1st, 2nd. and 3. measurements were made after sessions. Data from the study; the Patient Information Form, which includes socio-demographic and disease-related characteristics of hemodialysis patients, was collected with the Distres Thermometer and Visual Analog Scala. Identifying requests, dependent and independent sample t-testing, ANOVA was used in the analysis of data from the research. In order to evaluate the clinical significance of the difference between the groups, the partial eta squared (?p 2 ) value was calculated. The statistical significance level for all analyzes was accepted as p<0,05. Hemodialysis patients in the experimental group were found to have a significant decrease in VAS pain severity at the end of the third session compared to those in the control group (p<0,001). Patients in the experimental group were found to have reduced levels of distres according to the control group (p<0,001). It was found that music application did not change vital signs (p>0,05). According to the results obtained from the research a trusted nursing intervention to reduce levels of pain and distres associated with arteriovenous fistula canulation of music application in patients receiving hemodialysis treatment. This randomized controlled experimental trial is registered on ClinicalTrials.gov (2022/802).
