Öğe COVID-19 pandemisinin hastanelerin vaka karma indeksine etkisinin değerlendirilmesi: Tanımlayıcı bir araştırma(2022) Aslan, Emine Çetin; Orhan, MustafaAmaç: Araştırmanın amacı, koronavirüs hastalığı 2019 [coronavirus disease 2019 (COVID-19)] pandemisi döneminde, sağlık kurumlarının sunmuş oldukları sağlık hizmet üretim hacminin değişip değişmediğini araştırmaktır. Ayrıca COVID-19 hastalarının vaka karma indeksinin belirlenmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışma, retrospektif, kesitsel ve tanımlayıcı bir araştırmadır. Araştırma, Sağlık Bakanlığına bağlı bir B grubu hastanede yapılmış olup, veriler 2019 ve 2020 yıllarını kapsamaktadır. Sağlık hizmeti üretimindeki değişim tedavi edilen hasta sayısı, üretilen toplam teşhis ilişkili gruplar bağıl değeri ve vaka karma indeksi ile ölçülmüştür. Vaka karma indeksi, belli bir dönemdeki toplam bağıl değerlerin aynı dönemdeki toplam hasta sayısına bölünmesiyle hesaplanmıştır. Bulgular: Hastanede, 2019 yılında 15.944, ve 2020 yılında, 1.343’ü COVID-19 olmak üzere, toplam 10.697 hasta tedavi görmüştür. Dolayısıyla hasta başvurularında yaklaşık 1/3’lük bir azalış gerçekleşmiştir. Pandemi döneminde hastanede bakılan hasta sayısı ve üretilen toplam bağıl değer düşmüş, buna karşın vaka karma indeksi yükselmiştir. Hastanenin vaka karma indeksi 2019 yılında 0,90, 2020 yılında 0,97 ve COVID-19 hastalarında 0,99 bulunmuştur. COVID-19 hastalarının vaka karma indeksi dönemlere göre farklılık göstermiştir. Sonuç: Pandemi süreci hastanede üretilen sağlık hizmeti miktarının azalmasına neden olmuştur. Ancak vaka karma indeksi artmıştır. Vaka karma indeksindeki artışta, COVID-19 hastalarının teşhis ilişkili gruplar bağıl değerlerinin pandemi öncesi dönemde bakılan hastalarının bağıl değerlerinden yüksek olmasının yanında, düşük bağıl değerli vaka başvurularındaki azalışın ortak etkisinden kaynaklandığı sonucuna varılmıştır.Öğe Impact of COVID-19 on emergency service usage in Turkey: interrupted time series analysis(Springer-Verlag Italia Srl, 2023) Cirakli, Umit; Orhan, Mustafa; Sayar, Burak; Demiray, Emine Kubra DindarThe aim of this study is to reveal the impact of the COVID-19 pandemic, which constitutes an extraordinary situation, on the habits of emergency health service use. The data of the study consist of emergency service applications of a public hospital in Turkey between the years 2018-2021. The number of applications to the emergency service was examined periodically. The interrupted time series analysis method was used to reveal the impact of the COVID-19 outbreak on emergency service admissions. When the main findings are analyzed in quarterly periods (3 months = 1 quarter); there has been a sharp decrease in emergency service applications since March 2019, when the first case was seen in Turkey. When an evaluation is made between consecutive quarters, it is seen that there are fluctuations up to 80% in the number of applications. When the statistical analysis findings are examined; while the effect of COVID-19 on the number of applications was found to be significant for the first four periods, it was found to be insignificant for the following periods. With the conducted study, it was revealed that COVID-19 has a significant impact on the use of emergency health services. Although there was a statistically significant decrease in the number of applications, especially in the months following the first case, there was an increase in the number of applications over time. Considering the necessity of using emergency health services when necessary, it can be thought that some of the decrease in the number of applications during the COVID-19 period will be related to the use of unnecessary emergency health services.Öğe Systematic review on reengineering digital processes of healthcare institutions(2021) Orhan, Mustafa; Kafes, MustafaObjective: Organizations make changes on their processes in order to keep their costs under control while continuing their activities effectively and efficiently. This research has been carried out to evaluate the results of reengineering practices carried out within hospitals, based on cost and benefit themes. Material and Methods: Systematic review is an important method for revealing best practices and implementation results. The systematic review method was used to analyze and interpret the data of the research and to present the findings. The systematic review was made using the PRISMA flow chart. As a result of the first examination, 1931 studies were found and 13 studies that were suitable for the inclusion criteria were evaluated within the scope of the study. Results: Reengineering practices carried out in hospitals were examined under the themes of benefit and cost and it was seen that results were obtained from various perspectives. When considered in terms of benefit; while there are studies that have shown positive results such as ease of use, health output, increase in satisfaction and productivity, it is also seen that there are studies that have not shown any benefit. In terms of cost perspective, it has been observed that there are studies showing positive results in efficiency increase and saving. Conclusion: Although the investigated studies indicate efficiency, user-friendliness, increasing satisfaction, saving or increasing income as the main objective, multiple outputs have been obtained in one study. The systematic compile review study has revealed the best practice results, and it also provides suggestive for the reengineering studies to be carried out. With the studies examined, it can be said that the reengineering studies performed in technological processes yielded positive results.
